Common Causes of/Risk Factors for Lower Back Pain

Your back is an intricate highway of nerves, muscles, bones, tendons, ligaments, joints, discs, fascia, and blood vessels. These connective tissue structures work together to allow you to do everything from stand up to lift and carry objects. Sometimes, however, the system can start to break down and lead to pain, especially in the lower back. Here are a few of the most common reasons why:

  • Osteoarthritis
  • Nerve compression (including compression of the sciatic nerve, called sciatica)
  • Spinal stenosis
  • Degenerative disc disease
  • Disc herniation or disc bulge
  • Acute soft tissue injury (e.g., muscle strain or ligament sprain)
  • Joint locking or misalignment (including spondylolisthesis, where one vertebral bone slides forward relative to the one beneath it)

Other less common causes exist, including referred pain from organs, tumors, or secondary effects of other underlying health conditions. As mentioned, we're all at risk for having back pain at least once, but you may be more at risk if you have certain positive risk factors:

  • A history of lower back pain or injury
  • Tobacco use
  • Sedentary lifestyle
  • Overweight or obese
  • Mood and stress problems, including depression and anxiety 
  • Physically demanding job
  • Age between 30-50+

In addition to the back pain itself (often described by our patients as dull, achy, and sore), lower back pain also can present with other symptoms, too, including numbness, tingling, and weakness in one leg, stiffness in the lower back, decreased flexibility in the legs (especially the hamstrings and hip flexors), and muscle tenderness.
